Dear Library,
Remember the time my books were 6 days late and you charged me $3.00? Well, this time my books were 3 weeks late...
So I think the obvious thing to do here is break up. Actually, I will probably just stop calling you and stop coming around until you eventually forget about me. We're both better off this way, I promise. You got your books back, and I'll get to keep my $9. We'll call it even. Maybe someday we'll meet again...

pretty much the same thing happened to me. so i went in and got a new card with my new last name. haha. is that bad? oh well, did it anyway.
